Hello!  To help us make this day the best day ever, we would like to ask that you complete this form and return to My Tiny Wedding at contact@mytinywedding.com.  This completed form insures the staff at My Tiny Wedding that we have all the details that you want for your wedding day.  After receipt of this form we will send to you another email just before your wedding day, called “confirming the details” .  This email will have all that we have tying everything together as you have provided to us for your day.

How do you want us to handle your wedding guest using smart phones/cameras during the ceremony?*

We don't mind at all and welcome anyone to video and or take photos during the ceremony! (This will affect the photos that we take as any ceremony pictures that have the wedding guest in them may have them holding out their smart phones in front of them).
We would prefer that our wedding guest focus on our ceremony and the moment and leave the photography up to you. Please make a short announcement before the ceremony asking them to put their devices away and be fully there.

I completely understand that My Tiny Wedding is not responsible for lost guest as they have provided me with a custom website with detailed directions for my wedding guest and given them adequate information on the terrain of the area so they are well prepared.

I acknowledge that all of my guest are in good physical condition and able to hike the distance for this ceremony (three mile hike - Rock Bridge).
Any dog that I might be bringing is healthy and is good hiking condition.
I will bring plenty of water for my dog.
My guest will have proper shoes, water and headlamps for this hike.
I acknowledge that if me or any of my guest show up in improper foot attire or no water, we will need to switch locations to a shorter hike.

First and Last names of your two witnesses. These are the names that the officiant will be putting on the official license and must be people you are 100% positive will be attending your wedding. If you are not sure yet, that is ok (just put "not sure"), but you MUST remember to arrive to the ceremony site with a slip of paper on it with the witnesses names already written on it. (It's way easier for you to pick two people you know will be there now than to remember to print the witnesses names at the last minute, trust me). If this is an elopement with zero guest, simply type "you are providing".*
