Falling In Love “firming up the details” Form

Hello!  To help us make this day the best day ever, we will need a bit more information from you. We will need this form at least three weeks before but no more than five weeks before your wedding date.

Please do not send in the form months in advance. This creates more work for all of us later on. 


About 5 days before your wedding day, we will send you a “confirming the details” email tying everything together.

What is your final guest count? Please include children in this count. (we need to know this within 5 days of your ceremony so we can submit this information to the Forest Service)*

How will the two of you be dressed? Please check at least one (if you are both dressing the same) and no more than two (if you are each dressing differently)* (this will help the officiant determine how to dress to match).





Ceremony information: First, we need to know if you prefer a Religious or Non-Religious Ceremony (you must choose one of these two options). Next, check any other options that pertain to your ceremony. ***See note below for additional charges for Handfasting and God's Knot ceremonies. Any Special Readings for your ceremony (check out our ceremony link above for some cool ideas). If so, will you have a reader or do you want the Officiant to read?*

What is the story of your wedding day? How did you get to this point? What is the story of the two of you? Every once in awhile we post couple's stories on our blog/facebook or submit it to a wedding blog. We'd love to be able to add the story of the two of you to the pictures!

First and Last names of your two witnesses. These are the names that the officiant will be putting on the official license and must be people you are 100% positive will be attending your wedding. If you are not sure yet, that is ok (just put "not sure"), but you MUST remember to arrive to the ceremony site with a slip of paper on it with the witnesses names already written on it. (It's way easier for you to pick two people you know will be there now than to remember to print the witnesses names at the last minute, trust me). If this is an elopement with zero guest, simply type "you are providing".*
